exec('mysql --execute "GRANT ALL PRIVILEGES ON database.* TO 'user'@'122.169.XXX.XX' IDENTIFIED BY 'password'"');

    You have to escape the ':

    exec('mysql --execute "GRANT ALL PRIVILEGES ON database.* TO \'user\'@\'122.169.XXX.XX\' IDENTIFIED BY \'password\'"');
