    The HTML in your form is malformed.

    <form action="avvikelse.php" method="post">
    <p>Operation step: <input type="text" name="operation_step" /></p>
    <p>Problem detail: <input type="text" name="problem_detail" /></p>
    <input type="submit" value="Submit" />
    </form>
