    try this:

    $str = preg_replace('{^(<br(\s*/)?>|&nbsp;)+}i', '', $str); //from start $str = preg_replace('{(<br(\s*/)?>|&nbsp;)+$}i', '', $str); //from end 

    that also gets XHTML <br /> and <br/> forms
