    Jasny’s fork of Bootstrap allows you to do come close. See documentation.

    The code:

    <div class="fileupload fileupload-new" data-provides="fileupload">
      <div class="fileupload-preview thumbnail" style="width: 200px; height: 150px;"></div>
      <div>
        <span class="btn btn-file"><span class="fileupload-new">Select image</span><span class="fileupload-exists">Change</span><input type="file" /></span>
        <a href="#" class="btn fileupload-exists" data-dismiss="fileupload">Remove</a>
      </div>
    </div>

    It doesn’t seem to have a progress bar, unfortunately.
