I have always been told that "incoming call" screen is not customizable, because of apple’s controls. But this app has obviously modified the incoming call screen.

Just wonder how this is done, because this app is on app store, meaning we don’t need to root iOS.

From the way the article is worded, it sounds like the sync.me app scans your friends social network status updates and appends them to the picture you have saved on their contact info.
The call screen is not being modified. Just the picture associated with the contact.
