    On Form1:

        public partial class Form1 : Form
    {
        public Form1()
        {
            InitializeComponent();
        }

        private void checkBox1_CheckedChanged(object sender, EventArgs e)
        {
            Form2 a = new Form2();
            a.c = checkBox1.Checked;
            a.ShowDialog();
        }
    }

    On Form2:

        public partial class Form2 : Form
    {
        public bool c;
        public Form2()
        {
            InitializeComponent();
        }

        private void Form2_Load(object sender, EventArgs e)
        {
            checkBox1.Checked = c;
        }
    }
