Why is Refactor… grayed out (disabled) in Xcode?

Why is Refactor… grayed out (disabled) in Xcode?
I’d like to rename a class.
Select the class’s symbol in its header file – i.e. the bit just after
@interface. Then the refactoring stuff should be enabled.
